Yes. Many torn ligaments can heal without surgery. 

Grade 1 injuries and most Grade 2 partial tears are usually treated without an operation. Even some complete Grade 3 ligament tears can heal successfully with bracing and rehabilitation. However, whether a ligament will heal well depends on which ligament is torn, whether the joint remains stable, the severity and location of the tear, other associated injuries, and what activities you need to return to.

An ankle ligament tear, for example, is very different from a complete ACL tear inside the knee. Most acute ankle sprains — including many complete ligament tears — are managed without surgery, whereas a complete ACL tear causing knee instability in an active person is much more likely to require reconstruction.

This is why an MRI report saying “complete ligament tear” does not automatically mean you need surgery.

Let me explain how we decide.


What Happens When a Ligament Tears?

Ligaments are strong bands of connective tissue that connect one bone to another and help keep a joint stable.

A ligament injury is usually classified into three grades:

Grade 1 ligament injury

The ligament has been stretched and some microscopic fibres may be damaged, but the ligament remains intact.

There is usually:

  • mild pain

  • mild swelling

  • little or no instability

  • relatively preserved ability to use the joint

Grade 2 ligament tear

Part of the ligament has torn, but some fibres are still intact.

There may be:

  • moderate pain and swelling

  • bruising

  • difficulty walking or using the joint

  • some looseness or instability

Grade 3 ligament tear

The ligament has torn completely or has been pulled away from its attachment.

There may be:

  • significant swelling and bruising

  • difficulty bearing weight

  • loss of normal function

  • instability or a feeling that the joint is “giving way”

Grade 3 is therefore a complete ligament rupture. But even at Grade 3, treatment depends on the ligament involved rather than the grade alone.

Can Ligaments Heal Themselves?

Many ligaments have the ability to heal naturally.

After a ligament tears, the body begins a repair process. Bleeding and inflammation occur initially, followed by the formation of new collagen. Over the following weeks and months, those collagen fibres gradually reorganise and strengthen.

However, the healed tissue is not necessarily identical to the original ligament. Healing commonly involves the formation and remodelling of scar-like collagen tissue. This is one reason rehabilitation is so important — the ligament needs not only biological healing but also appropriate mechanical loading so that the joint regains strength and stability.

There is another important point:

A ligament can heal anatomically but the joint may still remain functionally unstable.

That distinction often determines whether surgery is eventually required.


Why Do Some Ligaments Heal Better Than Others?

Not every ligament has the same healing ability.

Location, blood supply and the environment around the ligament all matter.

For example, the medial collateral ligament (MCL) lies on the inner side of the knee and has good potential for healing. Most isolated MCL tears can therefore be treated without surgery.

The ACL, on the other hand, lies inside the knee joint. Its environment and relatively limited ability to form an effective healing bridge between the torn ends mean that complete ACL tears have traditionally had much poorer spontaneous healing than MCL injuries.

So when somebody asks me:

“Doctor, my ligament is completely torn. Do I need an operation?”

The first thing I would want to know is:

Which ligament?

The words complete tear alone are not enough to decide.


Which Torn Ligaments Can Heal Without Surgery?

Ankle ligament tears

The majority of ankle ligament injuries are treated without surgery.

This includes:

  • Grade 1 ankle sprains

  • Grade 2 partial ligament tears

  • many Grade 3 complete ligament tears

Treatment may involve short-term protection, an ankle brace or walking boot when necessary, followed by progressively increasing movement, strengthening and balance training.

Surgery for a straightforward ankle sprain is uncommon. It is more often considered when the ankle remains unstable despite adequate rehabilitation or when there are repeated sprains and chronic instability.

So even if an MRI shows a complete ankle ligament tear, surgery is not inevitable.


MCL tear of the knee

The MCL is one of the best examples of a ligament with good healing potential.

The vast majority of isolated MCL injuries can be treated without surgery. Even many complete MCL tears can heal with an appropriate hinged knee brace and rehabilitation.

Surgery becomes more relevant if:

  • the ligament has been torn away in a way that prevents normal healing

  • the knee remains unstable

  • several knee ligaments have been injured

  • there is another significant associated injury

  • appropriate nonsurgical treatment has failed


PCL tear of the knee

The posterior cruciate ligament, or PCL, also has more potential for healing than the ACL.

Many PCL injuries are partial tears and isolated PCL injuries can often be managed successfully with a specialised brace and physiotherapy.

Rehabilitation particularly focuses on restoring quadriceps strength and controlling posterior movement of the tibia.

Surgery becomes more likely with severe instability, persistent symptoms or injuries involving several knee ligaments.


ACL tear of the knee

The ACL is different.

Partial ACL tears with a stable knee can sometimes be managed without surgery using structured rehabilitation. AAOS guidance notes that recovery and rehabilitation for a partial ACL tear generally takes at least several months.

Some people with complete ACL tears can also function without reconstruction, particularly if they:

  • do not experience significant knee instability

  • do not participate in pivoting or cutting sports

  • perform relatively low-demand activities

  • are willing to modify their activities

However, a complete ACL tear causing repeated episodes of the knee giving way is much more likely to require reconstruction.

This is particularly relevant for people playing sports such as:

  • football

  • basketball

  • rugby

  • netball

  • tennis

  • squash

or those doing jobs involving frequent twisting, turning or heavy physical work.

Repeated instability after an ACL tear can also place other structures, particularly the meniscus and articular cartilage, at risk of further injury.

Does a Grade 3 Ligament Tear Always Need Surgery?

No.

This is probably the most important misconception surrounding ligament injuries.

Grade 3 tells us that the ligament is completely torn. It does not tell us automatically how it should be treated.

How Is a Torn Ligament Treated Without Surgery?

Nonsurgical treatment does not mean simply lying in bed and waiting for the ligament to heal.

Good rehabilitation is an active process.

1. Protect the injured joint

During the first few days, the injured area should be protected from movements that could worsen the tear.

Depending on the injury, this might mean:

  • reducing weight bearing

  • using crutches

  • wearing an ankle support

  • using a hinged knee brace

  • occasionally using a walking boot

For many soft-tissue injuries, protection, relative rest, ice, compression and elevation can help control pain and swelling during the early phase.


2. Control pain and swelling

Ice can be applied for short periods during the early stage, with a cloth between the ice and skin.

Compression and elevation may also reduce swelling.

Simple pain relief may be appropriate depending on your medical history and other medications. Medication should be used for comfort rather than as a substitute for protecting and rehabilitating the injury.


3. Avoid prolonged complete rest

One mistake is assuming that a ligament should remain completely immobilised until it has healed.

For many ligament injuries, once the acute pain and swelling have settled sufficiently, controlled movement and progressive loading are important.

Excessive immobilisation can contribute to:

  • joint stiffness

  • muscle weakness

  • loss of balance and proprioception

  • slower functional recovery

Severe injuries may require a short period of immobilisation, but rehabilitation usually progresses toward movement and strengthening as soon as it is safe.


4. Physiotherapy and rehabilitation

Physiotherapy is one of the most important parts of nonsurgical ligament treatment.

A rehabilitation programme may include:

Range-of-motion exercises

These restore normal joint movement and reduce stiffness.

Muscle strengthening

The muscles surrounding the injured joint provide additional stability while the ligament is healing.

Balance and proprioception exercises

Ligaments contain sensory receptors that help the brain understand the position of a joint.

After a ligament injury, this control can become impaired.

This is one reason someone may say:

“The pain is gone, but the ankle still doesn't feel right.”

Balance and neuromuscular exercises are therefore particularly important following ankle and knee ligament injuries.

Sport-specific rehabilitation

Running, jumping, cutting and pivoting should be introduced progressively rather than simply returning to sport because a certain number of weeks have passed.


How Long Does a Torn Ligament Take to Heal?

There is no single ligament-healing time.

Recovery depends on:

  • the ligament involved

  • whether the tear is partial or complete

  • joint stability

  • associated cartilage, meniscus, tendon or bone injuries

  • age and general health

  • smoking

  • previous injuries

  • rehabilitation

  • the activity you are trying to return to

As a very rough guide:

These are not fixed deadlines.

For example, a mild ankle sprain may improve within one or two weeks, while a complete ankle ligament injury may require many weeks or several months. NHS guidance similarly notes that most uncomplicated sprains start feeling significantly better within about two weeks, while severe sprains may take months to return to normal.

An isolated MCL tear may recover considerably faster than an ACL reconstruction even if both were initially described as “ligament tears.”

How Do I Know If My Torn Ligament Is Healing?

Signs of improvement usually include:

  • decreasing swelling

  • decreasing pain

  • improving range of movement

  • being able to bear more weight

  • increasing strength

  • improved confidence in the joint

  • fewer episodes of giving way

However, pain disappearing does not necessarily mean the ligament has regained full strength.

This is especially important for athletes.

You may feel comfortable walking long before the ligament, surrounding muscles and neuromuscular control are ready for sprinting, jumping or changing direction.

Returning too early increases the risk of another injury.


Can I Walk With a Torn Ligament?

Sometimes.

The ability to walk does not reliably tell us whether a ligament is torn.

People with some significant ligament tears can still walk, while an acute moderate sprain may initially be too painful to walk on.

Weight bearing should therefore depend on the injury involved and your clinician's advice.

Seek medical assessment if you are unable to bear weight, have severe swelling or bruising, significant instability or worsening pain.


Do I Need an MRI for a Ligament Tear?

Not every ligament injury needs an immediate MRI.

Many uncomplicated ankle and knee sprains can initially be diagnosed from:

  • how the injury happened

  • examination

  • joint stability testing

  • symptoms

X-rays do not normally show the ligament itself, but they may be needed to exclude fractures or ligament avulsion injuries where a ligament pulls off a piece of bone.

MRI becomes particularly useful when:

  • a significant ligament tear is suspected

  • the diagnosis remains uncertain

  • the joint is unstable

  • symptoms are not improving as expected

  • surgery is being considered

  • a meniscal, cartilage or other associated injury is suspected

For significant knee injuries, MRI can show the ligament as well as associated damage to other structures.


When Does a Torn Ligament Need Surgery?

Surgery becomes more likely when one or more of the following are present.

1. Persistent joint instability

This is one of the most important indications.

If the knee or ankle repeatedly gives way despite appropriate rehabilitation, the ligament may not be providing sufficient functional stability.


2. A ligament with poor healing potential

The classic example is a complete ACL tear causing instability.

ACL reconstruction is particularly considered in active individuals who want to return to sports or work involving pivoting, turning and cutting movements.


3. Multiple ligament injuries

A knee injury involving several major ligaments is very different from an isolated sprain.

Combined ligament injuries are much more likely to require surgical reconstruction.


4. Associated injuries

Surgery may be necessary because the ligament tear occurs together with another injury such as:

  • a repairable meniscus tear

  • significant cartilage injury

  • fracture

  • displaced avulsion fracture

  • tendon injury

  • joint dislocation


5. Failure of appropriate rehabilitation

Sometimes conservative treatment is reasonable initially, but the joint remains painful or unstable after a proper rehabilitation programme.

For example, surgery for ankle ligament injury is generally uncommon but may be considered when the ligament fails to heal adequately and chronic instability persists despite physiotherapy.


6. The patient's functional requirements

A 25-year-old competitive football player and a 65-year-old person who mainly walks for exercise may make very different treatment decisions despite having similar MRI appearances.

The correct question is therefore not simply:

“How badly is the ligament torn?”

It is also:

“What does this person need this joint to do?”


What Happens If a Torn Ligament Is Left Untreated?

A ligament tear that is suitable for conservative treatment is not really being “left untreated.”

Bracing, progressive loading and rehabilitation are treatments.

The concern arises when a significant unstable ligament injury is ignored.

Possible consequences include:

  • recurrent giving way

  • repeated sprains

  • chronic joint instability

  • muscle weakness

  • altered movement patterns

  • secondary cartilage or meniscal damage in some injuries

  • difficulty returning to sport

For example, chronic instability following ACL injury can increase the likelihood of further meniscal and cartilage damage.


Can a Ligament Heal Loose?

Yes.

A ligament may heal but remain longer or more lax than it was before the injury.

If the surrounding muscles and neuromuscular control compensate adequately, this may cause very little functional problem.

In other people, the looseness results in recurrent instability.

This is another reason why MRI appearance alone does not determine recovery.

What matters ultimately is whether the joint is:

  • stable

  • strong

  • pain-free

  • functioning normally for the activities you need to perform


Can You Speed Up Ligament Healing?

There is no treatment that can make a significant ligament tear heal overnight.

The most useful things are relatively simple:

  • protect the injury appropriately

  • avoid repeatedly reinjuring it

  • follow the prescribed brace or weight-bearing plan

  • regain movement progressively

  • complete strengthening exercises

  • work on balance and proprioception

  • eat an adequate balanced diet with sufficient protein

  • avoid smoking

  • sleep adequately

  • progress back to sport gradually

One of the biggest mistakes is repeatedly testing the ligament before it is ready:

“It feels better today, so I'll try running.”

Then the joint swells again.

A good rehabilitation programme progresses according to pain, swelling, strength, movement and stability, not simply the number of days since the injury.


My Practical Approach to a Ligament Tear

When I explain ligament injuries, I think it is more useful to answer three questions than simply concentrate on the MRI report.

1. What exactly is torn?

An ACL, MCL, PCL and ankle ligament do not behave in the same way.

2. Is the joint stable?

A complete tear in a functionally stable joint may sometimes be managed without an operation.

A tear producing repeated giving way is more concerning.

3. What does the patient need to return to?

Walking comfortably around the house and returning to competitive football place completely different demands on a ligament.

That is why two people with apparently similar MRI reports can quite reasonably receive different treatment recommendations.

We treat the patient and the function of the joint — not just the words written on the MRI report.


When Should You See a Doctor After a Ligament Injury?

Get the injury assessed if you have:

  • severe pain

  • rapidly increasing swelling

  • extensive bruising

  • inability to bear weight

  • inability to move the joint normally

  • a joint that repeatedly gives way

  • a significant popping sensation followed by swelling

  • visible deformity

  • persistent symptoms that are not improving

Seek urgent medical attention if the limb becomes:

  • numb

  • cold

  • pale or blue

  • obviously deformed

These signs can suggest a more serious injury than a simple ligament sprain.


Frequently Asked Questions

Can a partially torn ligament heal without surgery?

Yes. Most Grade 1 injuries and many Grade 2 partial ligament tears are treated without surgery using protection, progressive rehabilitation and physiotherapy.


Can a completely torn ligament heal without surgery?

Sometimes.

Many complete ankle ligament tears and isolated MCL tears can recover without an operation. Complete ACL tears have less predictable healing and are more likely to require reconstruction when they cause instability or when the patient wants to return to pivoting sports.


Does a Grade 3 ligament tear need surgery?

Can a ligament grow back together?

Many ligaments can reconnect through the body's healing process, producing new collagen and scar-like repair tissue.

However, the healed ligament may not have exactly the same structure or mechanical properties as the original ligament.


Is physiotherapy necessary for a ligament tear?

For significant ligament injuries, rehabilitation is extremely important.

Physiotherapy helps restore:

  • joint movement

  • muscle strength

  • balance

  • proprioception

  • joint stability

  • confidence

  • safe return to sport

Simply waiting until the pain disappears does not restore all of these functions.


Should I wear a brace for a torn ligament?

A brace can be useful for certain injuries, particularly some ankle, MCL and PCL injuries.

However, the type of brace and how long it should be worn depend on the ligament involved. Wearing an unnecessary brace for too long can also contribute to stiffness and weakness.


Can I exercise with a torn ligament?

Usually rehabilitation exercises are introduced progressively, but unrestricted exercise should not be restarted simply because the pain has improved.

Return to sport should ideally occur when you have regained appropriate:

  • movement

  • strength

  • balance

  • stability

  • sport-specific function


The Bottom Line

Many torn ligaments can heal without surgery — including some complete tears.

Grade 1 and Grade 2 injuries are commonly treated conservatively. 

Many ankle ligament tears, MCL tears and selected PCL injuries also have good potential for nonsurgical recovery.

The major exception is when the injured ligament has poor healing potential or the tear leaves the joint functionally unstable — a common concern with a complete ACL tear in an active person.

The decision about surgery should therefore consider:

the ligament involved + severity of injury + joint stability + associated injuries + your activity requirements + response to rehabilitation.


So if an MRI report says “complete ligament tear,” don't assume that surgery is inevitable.

Equally, don't ignore a joint that repeatedly gives way simply because the pain has improved.

The goal of treatment is not merely to make the scan look better.

The goal is to restore a stable, strong and functional joint.
